To provide the participants with the most recent information about:

  • Bio-impedance
  • Adjuvant Systemic Therapies for Breast Cancer
  • Surgical  Treatment of Breast Cancer .
  • To give an opportunity to the participants to ask questions to the lecturers

    Early Registration: $200.00 (Until 08/15/2010)

   Regular Registration: $250.00 (Until 09/12/2010)
   
    Late Registration: $300.00 ( After 09/12/2010)

 

Cancellations and requests for refund will be accepted until September 23, 2010 minus a $70.00 administrative fee.

There are three ways to register through the Registration page:
 
1. Online registration:   entails filling out the electronic form contained in the Registration page.  This method guarantees receipt of your information within 12 hours, however, a check or money order will still have to be received in order to complete your registration.

2. The second way to register for the course is by printing the manual fill-out form page, filling out the information requested on it and mailing it with a check or money order enclosed. 

3. This third alternative is meant for use if you are experiencing problems in registering through the other two methods,  we will be happy to accept your information in the form of a note (typed or clearly written) sent in with your payment. In order to register you we need the following information:
Your Name, Home Address, your Phone and Fax numbers, your Facility's Address, its Phone and Fax number. Your title: PT, OTR...and License number.
